The list of current medications helps maintain the accurate names & dosages, especially if there are frequent changes.

Maintaining and updating current and historical healthcare issues, procedures, and surgeries aids both patient's and provider's awareness of a possible wide variety of treatment over time.

Some patients may have multiple different surgeries that could impact each other. And medications sometimes change often.

A magnet is on the back of the Passport so that it can be kept in an easily visible area in the home. If the patient cannot communicate, the emergency responders will have the information by taking the Passport with them.

Providing an extra completed Passport to appropriate significant persons who aid with a patient's care gives them the current and accurate information.
